Well I can't be totally sure about the specifics in your case, but maybe if you knew exactly how it works you could figure it out yourself.
The index.html file is just the page that displays the canvas. It references the folder (html5game) which contains the main Javascript code and images. Theoretically if you create a zip folder and put index.html and the html5games folder inside, after uploading it to a game portal they will unzip it and it will run. Remember that it must be running from a server to work properly, so testing it on your local machine simply by opening index.html in a browser will not work.

GMS will also create a folder called html5games, place that folder and index.html into a zip folder and submit the zip folder.
